Inspector Notes

A mandated renewal inspection was conducted on July 21, 2023 from approximately 10:30am-11:45am. There were 26 children in care with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed actively engaged in imaginative play, following painting instructions, manipulating magnets, and building with blocks. A teacher was observed sitting at a small table with 2 other children engaged in imaginative play lunch. A teacher was observed dancing with the children to songs being encouraging, singing, dancing and doing the actions to the song. Teachers addressed the children in both English and Spanish. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, background checks were not repeated every five years for a applicant for licensure as a child day center.

Evidence:
1) Staff 1 has a central registry search which was completed on 04/27/2018 and a fingerprint based criminal history search completed on 07/17/2018.
2) Staff 2 has a fingerprint based criminal history search completed on 04/22/2018.
Plan of Correction: We will obtain updated background checks immediately.
Standard 8VAC20-780-160-C
Based on record review, staff did not undergo a follow up tuberculosis screening two years from the date of the most recent screening or test.

Evidence:
1. Staff 1 has a tuberculosis screening which was last conducted on 1/20/20.
2. Staff 2 has a tuberculosis screening which was last conducted on 8/5/21.
Plan of Correction: Updated tb screenings will be obtained immediately.

Based on record review, the center did not ensure all staff completed orientation training no later than seven days of their start date.

Evidence:
1) Staff 3 does not have a record of completing orientation. (Staff 3, DOH 7/3/23)
2) Staff 4 does not have a record of completing orientation. (Staff 4, DOH 4/27/23)
3) Staff 5 does not have a record of completing orientation. (Staff 5, DOH 5/11/23)
