During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/20/2025. The onsite inspection began at 10:30am and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-140-A
The center is required to obtain documentation of a physical examination by or under the direction of a physician for each child within 30 days after the first day of attendance. The record for Child #2 did not contain documentation of a physical examination. The documentation is overdue by one month.
Plan of Correction: The center will ask the parent for a current physical for Child #2.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. The updated screening for Staff #1 was due 4 months ago and the updated screening for Staff #5 was due more than one year ago. The updated screenings have not yet been completed.
Plan of Correction: Staff will complete updated tb screenings.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2, enrolled for approximately two months, was missing documentation of the child's first date of attendance, and documentation of viewing proof of the child's identity and age. The record for Child #3, enrolled for two months, was missing the phone number for 1 of 2 required designated people to call in an emergency if a parent cannot be reached. The records for Child #4, enrolled for over one year, and Child #5, enrolled for 6 months, was missing documentation of viewing proof of the child's identity and age.
Plan of Correction: The center will request needed information from the parents and update the children's records.
Standard 8VAC20-780-70
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #1, who has been actively working at the center for more than one year, did not have any completed reference verifications on file. Documentation of reference verifications for Staff #3 did not contain the date the references were verified.
Plan of Correction: The center updated staff's record to include the date reference verifications were completed.

Repeat Violation.
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The written attendance for the Rabbits classroom was not complete. There were 7 children present.
