During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on 7/31/2025), as a part of the licensure period. The on-site inspection began at 9:10 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were (27) children present and (6) staff. The inspector reviewed (5) children?s records and (6) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
The center is required to have applicants undergo a background check every five years.
The most recent fingerprint results for staff #5 was overdue by 2 years and 1 month.
Plan of Correction: Applicant will schedule appointment for fingerprinting, results will be emailed to inspector.
Standard 8VAC20-780-350-F
The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
The center has not developed a written policy to ensure children receive care by consistent staff.
Plan of Correction: Director will complete a policy on consistent staffing.
Standard 8VAC20-780-40-K
The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
The center did not have a written procedure for abusive head trauma.
Plan of Correction: Director will develop a policy for head trauma.
Standard 8VAC20-780-520-C
When diaper cream is used, the center must follow the requirements.
There was no written parent authorization for 3 children's diaper creams in the Rabbits classroom. (child #6, 7 and 8)
These diaper creams were found under the changing table, accessible to the children in care.
Plan of Correction: Diaper creams were removed from changing table and place inaccessible to children during inspection.
Parents will complete authorization forms.
