Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated and concluded on 10/7/21. There were 21 children present with 8 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on records review, the center did not obtain results of a tuberculosis (TB) screening for employees within 30 days prior to the first day of employment.
Evidence:
1. Staff # 1's first day of employment was 8/12/21. Staff # 1's TB screening date was 8/18/21.
2. Staff # 2's first day of employment was 5/21/21. Staff # 2's TB screening date was 8/13/21.
3. Staff # 3's first day of employment was 7/30/21. Staff # 3's TB screening date was 5/20/21.
4. Staff # 4's first day of employment was 5/23/21. Staff # 4's TB screening date was 3/15/21.
Plan of Correction: All employees TB test will be taken with in 30 days of start date of employment
