Inspector Notes

An in person complaint inspection was initiated on 12/10/21 and concluded on 12/14/21. A complaint incident was received by the department regarding allegations in the areas of supervision, programming, behavior management, injury reporting and nutrition.
All classrooms were visited during the inspection, and 5 staff members were interviewed. 1 child's record and the injury report log was reviewed. The evidence gathered during the investigation supported the allegation of non-compliance with standards. Violations related to the complaint have been cited on the Violation Notice. Standard 8VAC20-780-450-A
Based on staff interview and records review, cots used by children did not have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence:
1. On 9/20/21, Child # 1 slept on his cot with his jacket on and without any linens.
2. Documentation on the program used by the center to update and communicate with parents, indicated that Child # 1's linens were not at the center, and the center did not provide extra linens to use for Child # 1's cot during nap time.
3. Staff # 1 stated that Child # 1's linens had not been brought back from home, and due to COVID 19 the center did not maintain an extra supply of linens.
Plan of Correction: The center will keep clean emergency sheets on site for use.
