Inspector Notes
An unannounced complaint inspection was conducted from 11 am to 1:45 pm, related to a complaint received on 6/3/22. The complaint was related to children's injuries, and injury reports. 5 staff were interviewed, two staff records, one child's record, attendance records and the center's policies were reviewed. Classroom observations were made to assess supervision and ratios.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice. Violations
3Evidence:
1. Staff # 3 and Staff # 5 stated that Staff # 3 is the program lead for the Discovery Preschool classroom(2 years old).
2. Staff # 3's record only had documentation of a high school diploma. There was no other documentation in Staff # 3's record to show that she was qualified to be a program lead.
Evidence:
1. On 5/26/22, the Child Supervision Record (CSR) for the Discovery Preschool classroom (2 yrs old) had written documentation of 16 children in care, including Child # 1 (age: 16 months), Child # 2 (age: 19 months), and Child # 3 (age: 20 months).
2. The teacher/staff attendance portion of the CSR showed that Staff # 1 left for break at 10:50 am. At that time Staff # 2 and Staff # 3 were in the classroom to do lunch and put the children down for nap.
3. Staff # 1 and Staff # 2 stated that lunch was at approximately 11:30 am, and nap time started at approximately 12pm.
4. From 10:50 am to 12pm, the classroom was out of ratio with one child. The ratio of 1 staff to 5 children was not maintained.
Evidence:
1. On 6/6/22 Child # 4 (age 2 years old) in the Discovery Preschool classroom, sustained scratches on her face which resulted in broken skin and some bleeding.
2. Staff # 4 was notified by Staff # 2 and Staff # 3. Staff # 4 wrote and signed the accident report, but the report was inaccurate and missing information.
3. The date of the scratches was 6/2/22. The date documented on the accident report was a future date of 6/30/22.
4. The report was also missing the time that the parent was notified, and two staff signatures or one staff and one parent signature.
