Inspector Notes

A monitoring inspection was conducted on 2/14/23 from 10:50 am to 12:55pm. There were 31 children present, with 6 staff supervising. The children were observed during structured play, diaper changes and being fed. 7 staff records, 2 children's records and emergency drill records were reviewed. There were no medications on site.

Standard 8VAC20-780-160-A
Based on records review, each staff member did not submit a negative tuberculosis (TB) screening by the first day of employment and prior to coming into contact with children.
Evidence:
1. Staff # 2's first day of employment was 8/1/22.
2. The TB screening in Staff # 2's record had already been expired on 2/17/22.
3. A current and negative TB screening was not obtained for Staff # 2 prior to coming into contact with children and prior to the first day of employment.
Plan of Correction: A current TB will be obtained.
Standard 8VAC20-780-160-C
Based on records review, results of a Tuberculosis (TB) screenings were not obtained from each employee every two years.
Evidence: Staff # 1's most recent TB screening expired on 5/30/21. A subsequent screening result was not obtained after that date.
Plan of Correction: A current TB screening was emailed to the licensing inspector.
