Inspector Notes
An unannounced non-mandated inspection was conducted on 2/7/24 from 10:50 am to 1:10pm. This inspection as conducted to determine the center's compliance during the provisional licensure period. There were 27 children in care of 5 staff members. The children were observed playing outdoors and eating lunch. The lunch menu included chili, corn muffins, cheese, cole slaw and milk. 5 staff records, 2 children's records, child supervision records and other daycare postings were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Violations
3Evidence:
1. Staff # 1 stated that Staff # 2 was the program leader for the Preschool (3 yr. old) classroom.
2. Staff # 2's record did not have documentation of educational qualifications that would meet the requirements for a program lead.
Based on observation, areas inside and outside the center were not maintained in a safe condition.
Evidence:
1. Two metal window frames on the 2's playground had sharp and rusty edges protruding, posing an injury hazard to the children.
2. The wooden window frames around the windows facing the playground in the 2's classroom has splintering wood and two protruding nails, posing an injury hazard to the children.
Evidence:
1. Staff # 2's first date of employment was 1/10/24.
2. Staff # 3's first date of employment was 1/5/24.
3. Documentation of reference checks were not obtained for Staff # 2 and Staff # 3, and were not available for review during the inspection.
