During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on May 12, 2025. The on-site inspection began at 10:25am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 72 children present and 13 staff. The inspector reviewed 7 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and 3 (Use Zones for Equipment) of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. The resilient surfacing mulch on the outdoor playground is measuring approximately 2.5 inches in depth and at the end of the slides, the landing area had areas of dirt and minimal mulch.
Plan of Correction: Work order for mulch was placed during the inspection and expected to be completed within 30 days. Going forward, Safety Captain will notify management when resilient surfacing measures less than 6" and management will address immediately with facilities manager.
Standard 8VAC20-780-440-G
Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. Child #1, age 15 months, sleeps in a crib in the infant room and has no noted developmental delays or special sleep accommodations in their record. .

Shelter in place procedures shall be practiced a minimum of twice per year. There is no documentation that shelter in place procedures were practiced at the center in the past year.
Plan of Correction: Shelter in place drill for 2025 completed on 5/20/25 and school management has planned next drill for November 2025. Drill logs will be completed and sent to DM monthly for monitoring of completed drills to ensure compliance.

For staff, documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff #1 did not have reference checks completed and has been working in the center with children for one month.
Plan of Correction: Reference check was completed the day of inspection. All reference checks will be completed prior to employee's start date and updated on management tracking form to ensure compliance.
