During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 7/16/2025 and completed on 7/16/2025 in response to complaints received by the licensing office on 6/20/2025 and 6/30/2025. The allegations involved the areas of Administration, Physical Plant, Special Care Provisions and Emergencies, and Access to Minor's Records as outlined in the Standards. The inspector reviewed ten children?s records and seven staff records on-site. This inspection included document review, interviews, and a tour of the facility.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Though not a part of the original complaint, violations that were not reported were also found related to the standard 8VAC20-780-80 involving reports and are documented on the violation notice issued to the program

Hazardous substances such as cleaning materials must be kept in a locked place using a safe locking method that prevents access by children.

A can of disinfectant and a container of disinfecting wipes with warning labels, ?Keep out of reach of children,? were stored unlocked on a counter top and a low shelf in room 123. Rooms 117 and the multipurpose room had disinfecting wipes stored unlocked on top of a cabinet.
Plan of Correction: Containers with locks have been ordered to store cleaning supplies in a locked place and out of reach.
Standard 8VAC20-780-80-B-2
The center must inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

The center did not notify the department's representative about an incident that occurred on June 18, 2025, in which Child #1 required outside medical attention after picking up and eating another child's food during mealtime.
Plan of Correction: Going forward, we will inform the department's representative within two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
