During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on December 17, 2024, and completed on December 17, 2024, as a part of the licensure period. The on-site inspection began at 10:45 am and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 150 children present and 28 staff. The inspector reviewed ten children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #2 completed the course four months late.
1. Tape had been applied to a seem on the outdoor track. The tape was rolling up and was detached around the edges posing a tripping hazard.
2. The Plexi-glass end piece of a crib was cracked at the bottom corner. The crack formed a circular shape and caused the center to be raised and able to be pushed out if pressed on exposing a sharp edge.
1. Called company, rep. coming out to replace plexi-glass or will replace crib if needed.
2. Building maintenance repaired tape on track while inspector was on site.
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization needs to be completed.
Medication for child #13 was onsite, however the medication authorization had expired over three months ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.
