During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 23, 2025, and completed on April 23, 2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 12:10 pm. The inspectors reviewed compliance in the areas listed above. There were 164 children present and 26 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-P
Repeat Violation
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child #16 expired two months ago. The medication was on site.
Plan of Correction: Per the director: Will email parent to get updated medical authorization form.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. 1. The injury report for child #15, dated 01/28/2025, did not contain the time parents were notified of the injury. 2. The injury report for child #19, dated 01/16/2025, did not contain the time parents were notified of the injury. 3. The injury report for child #20, dated 03/04/2025, did not contain the time the parent was notified of the injury and documentation on how the parent was notified.
Plan of Correction: Per the director: I will remind teachers in weekly email to complete the injury report forms to include actually times and how parents notified.
