The center shall obtain a complete sworn statement prior to the employee's first
day of employment. The record of staff #5 contained a signed sworn statement dated one day after the date of employment.

Centers shall obtain a completed national criminal fingerprint background check prior to the employee's first day of employment. The record of staff #4 contained the results of a national criminal fingerprint background check dated two days after the date of employment.

Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days prior to employment and be signed by a physician, physician's designee, or an official of the local health department. 1. The record of staff #5 contained documentation of a tuberculosis screening dated seven days after the date of employment. 2. The record of staff #7 contained documentation of a tuberculosis screening dated 14 days after the date of employment. 3. The record of staff #8 contained documentation of a tuberculosis screening dated 11 months prior to the date of employment.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Prescription medication for child#15 was not labeled with the child?s name. dosage amount and time or times to be given.
