A subsidy health and safety inspection (SHSI) was conducted on3/29/2023 with the executive director. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined noncompliance with subsidy regulations and was documented on the violation notice issued to the vendor.

Based on review of documentation and interview with staff, it was determined that the vendor failed to report an injury to a child while under supervision of the center that required outside medical treatment within two business days. Evidence: On 11/2/2022, Child #1 sustained an injury to their teeth, lip and mouth and required an emergency room visit and medical treatment and the vendor did not report the occurrence within two business days.
