Inspector Notes

A complaint was received in the licensing office on April 13, 2023 regarding the classroom being over-ratio and the classroom being overcrowded. On April 14, 2023 between the hours of 7:50am and 11:15am, an on-site investigation was conducted. Interviews were conducted, attendance sheets and child records were reviewed, and observations were made. Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, it was determined operational responsibilities of the licensee did not ensure that the terms of the current license issued by the department were compliant.
Evidence:
1. Child #1(date of birth 4/16/2021) was not 2 years of age at the time of enrollment. The child was 23 months on the first day of attendance (4/4/23).
2. The Director confirmed Child #1 was 23 months and 2 weeks on the first day of attendance.
3. The attendance sheets for the week of April 3-7, 2023 confirmed Child #1 first day of attendance was 4/4/23.
Plan of Correction: The Director stated "I thought a child could start if they were two weeks from the 2 year old birthday. The child will be 2 this weekend. I will not let children start before they are 2 years of age. "
