Inspector Notes
An unannounced monitoring inspection was conducted on October 18, 2023 and concluded on October 20, 2023. Ten staff were present supervising fifty-five preschool age children. Additional staff were on site for administrative duties. Children were observed eating lunch and preparing for nap time. Five staff and five child records were reviewed. Violation found is noted on the Violation Notice.

Based on review, facility did not ensure 3 of 5 staff have acceptable forms of documentation of tuberculosis screening.
Evidence: The records for staff #2, staff #3 and staff #4 had documentation of a tuberculosis screenings that were signed by a RN however it was not a physician, physician's designee or an official of the local health department as required.
