Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:30am-11:15am. There were a total of 51 children with 10 supervising staff members present. Children were observed during circle time, handwashing, and engaged in group activities. A selection of staff and children records, and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Based on record review, the center did not obtain an out of state sex offender registry search prior to the first day of employment.
Plan of Correction: The center conducted an out of state sex offender check during the time of the inspection.
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Pre-K classroom, the cushion of the sofa had rips in the right corner and the material was cracked, exposing the form of the cushion.
2. In the two's classroom, the wood on the top of the toy shelf contained an inch and a half crack.
Plan of Correction: The center had the couch removed from the classroom at the time of the inspection.

Based on record review, documentation for emergency evacuation drills did not contain required information.
Evidence:
1. Documentation for the emergency evacuation drills did not contain the following: number of staff participating, the method for notification, special conditions simulated, weather conditions, and problems encountered, if any.
Plan of Correction: The center will use the department's model form for evacuation drill documentation.
