Inspector Notes

An unannounced monitoring inspection was conducted on November 15, 2023 between the hours of 9:00am-11:30am. There were a total of 55 children with nine supervising staff members present. Children were observed during circle time, handwashing, coloring, reading books,engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the school age classroom, the sofa located in the library area had rips and tears in the seat cushion that exposed the fiber filling.
2. In the school age classroom, the red chair located in the writing center had a hole and a rip on the left side of the cushion. The green chair had a hole on the right side of the chair.
Plan of Correction: The chairs and sofa were thrown away at the time of the inspection.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the infant classroom, two bottles of graffiti remover were located in an unlocked low cabinet.
Plan of Correction: Staff locked the cabinet.
Standard 8VAC20-780-330-B
Based on observation, the center did not ensure that that where play equipment is provided, resilient surfacing complied with minimum safety standards.
Evidence:
1. The play equipment on the small playground did not have six inches of resilient surfacing throughout the fall zones. The center had approximately 2 inches of wooden mulch and decomposed mulch in the fall zones and under the slide. The ground was exposed under the slide.
Plan of Correction: A work order was placed for mulch for the playground.
Standard 8VAC20-780-510-G
Based on review of medications and interview, medication was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
1. Child #6 has emergency medication at the center. The emergency medication was located in a locked box in a plastic bag with their name on the bag. The emergency medication did not have the original container with a prescription label attached.
2. Staff #6 confirmed that Child #6's emergency medication did not have an original container with a prescription label.
Plan of Correction: The parents were contacted to get the original container with a prescription label for the emergency medication.
Standard 8VAC20-780-570-E
Based on observation, bottles were not dated and labeled with the child's name.
Evidence:
1. Two bottles of prepared infant formula located in the refrigerator of the infant classroom were not labeled with the child's name and date.
Plan of Correction: Staff labeled the bottles with the child's name and date at the time of the inspection.
