Inspector Notes
An unannounced monitoring inspection was conducted on May 29, 2024 between the hours of 9:45am-12:00pm. There were a total of 61 children with 11 supervising staff members present. Children were observed during centers, circle time, drawing, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
3Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1.The Little Tikes Playground Structure located on the small playground was covered in dirt and moss.
2.In the Pre-school classroom, there was peeling paint along the edge of the window ledge.
Based on observation, the center did not ensure that that where play equipment is provided, resilient surfacing complied with minimum safety standards.
Evidence:
1. The play equipment on the small playground did not have six inches of resilient surfacing
throughout the fall zones. The center had approximately 2 inches of wooden mulch and
decomposed mulch in the fall zones and under the slide. The ground was exposed under the slide.
Based on documentation review, the center did not obtain written authorization from the parent and physician to administer a medication.
Evidence:
1. Child #6's has epinephrine at the center to be administered in an emergency. Child #6's authorization on file expired on 03/06/2024 and does not have a current authorization.
