Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on April 8, 2025, as a part of the licensure period.The on-site inspection began at 9:45am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 57 children present and 13 staff. The inspector reviewed five children?s records and five staff records on-site.This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 16, 2025. A POC submitted after this date will not appear on the public website.
Violations
3Each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Child #2 did not have documentation of a physical examination in their record. The child has been in care for five months.
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Staff #5 did not have documentation of a negative tuberculosis screening at the time of employment. Staff #5 has been employed for one day.
Prepared infant formula shall be dated and labeled with the child's name.
Two of Child #6's bottles were not dated. Three of Child #7's bottles were not dated.
