Children under 10 years of age always shall be within actual sight and sound supervision of staff.
Two children, ages 2 years old, were observed walking into a hallway restroom without sound supervision. Staff #1 was standing in the doorway of Classroom A and actively supervising Classroom A in which was out of ratio by one, without a method of being able to hear the children in the restroom. The children were out of sound supervision in the hallway restroom for two minutes.

Plan of Correction: Restroom procedures have been revised to ensure a staff member accompanies children to the bathroom. Continue to hire additional staff to ensure to have support and the center remains fully staffed.
