Inspector Notes
An unannounced monitoring inspection was conducted on 04/25/2022 from 10:25 am - 1:12 pm. At the time of entrance there were 57 children in care with 10 staff supervising. The children ranged in age from infancy - five years. Children were observed during outdoor play, circle time and nap. Diapering was observed in the infant class. Records reviewed for four children and four staff.
Violations were found in the areas of physical plant and administration. These violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.
Violations
3Evidence:
1. Four areas of exposed landscape fabric was observed on the preschool playground.
a. This is a trip hazard.
2. An exposed tree root , measuring approximately seven feet, was observed near the sidewalk area of the preschool playground.
a. This is a trip hazard.
3. A fold up step ladder, measuring approximately four feet in height, was observed stored against the wall - beside a toilet in the three-year old bathroom.
a. Injury could occur if the ladder were to fall on a child.
4. Two pairs of adult scissors and three pieces of wrapped candy were observed in an unlocked drawer in the two-year old classroom. This drawer was accessible to the children in this class.
a. The adult scissors are not safe for children in this age group and the candy is of the size to be a choking hazard for children under the age of four years.
5. Standing water was observed inside of the water play table located outside near the Pre-K classroom door.
a. Standing water is a breeding ground for insects.
Evidence:
Grass and weeds were observed growing in the mulch around the outer fall zone perimeter of the two-year old outdoor play structure.
Evidence:
1. A current allergy list was not available in each classroom where children were in care during the inspection.
2. The Program Director confirmed that an allergy list was not on file in any of the classrooms.
