An unannounced complaint inspection was initiated on 04/18/2024 at 12:15 pm and concluded on 04/18/2024 at 4:45 pm in response to a concern regarding individual assignment of children's pacifiers.
Based on the information gathered this complaint is found to be valid. The violation is listed on the violation notice issued to the facility.

Based on staff interviews, the center failed to ensure that personal articles shall be individually assigned.
Evidence:
1. Staff 2 stated there was no system in place to ensure that infants pacifier's were individually assigned when she began working at the facility in late February 2024.
a. Staff 1, who is a caregiver in the infant classroom, confirmed there was no system to ensure that pacifiers were individual assigned.
b. Staff 1 stated that that on one occasion, the first week of March 2024, she observed a child with another child's pacifier in his mouth.
Evidence:

Plan of Correction: Infant room has individually labeled cubbies where children's pacifiers are kept when not in use. We have also labeled the actual pacifiers. Moving forward, we will continue to remain in compliance by ensuring personal items are individually assigned.
