Inspector Notes
An unannounced complaint inspection was initiated on 04/18/2024 with an onsite inspection. The inspection was completed on 04/30/2029 where center video was reviewed at the administrative office for the facility.
The complaint alleged non-compliance in the areas of care of children, activities for children and staff-to-child ratio. Based on the information gathered which included staff interviews, a review of center video and review of the facility the allegations regarding staff-to-child ratio and children's activities could not be substantiated therefore these concerns are found to be not valid.
The allegation regarding care of children is found to be valid and a violation is listed on the violation notice issued to the facility.
Violations
1Evidence:
1. Staff 2 stated that she observed staff 1, who was caring for children in the school age classroom state, "I quit," and then pick up a sealed bottle of Gatorade and throw it at the wall.
a. Staff 2 stated the children in the classroom appeared startled.
b. Staff 2 stated she felt she needed to get staff 1 "away from the kids" therefore she intervened and asked staff 1 to come to the office with her.
c. While enroute to the office staff 2 indicated that staff 1 "shoved a cart" sitting in the hallway, into the kitchen and then proceeded to enter the office where he began "throwing stuff."
d. Staff 2 stated that staff 1's demeanor was "agitated and angry" therefore, she took him outside and offered to call a spouse or relative to come and take him home.
e. Staff 1 declined assistance and stated he needed to take the children to school. Staff 1 proceeded to reenter the facility and gathered multiple children that he transported to school in the facility bus.
2. A review of center video for 04/18/2024 at approximately 7:36 am shows staff 1 throwing a plastic bottle of a liquid beverage at the wall of the school age classroom.
a. The bottle is thrown up agains the wall in a sudden and forceful manner.
b. The children in the classroom appear startled and surprised.
3. Staff 3 stated that when she arrived at approximately 8:00 am on 04/18/2024 the office was in disarray and she learned from staff 2 that staff 1 was "upset" and had thrown a bottle of Gatorade up against the wall of the school age classroom. She additionally was told that staff 1 had thrown objects around the office afterward.
a. Staff 3 confirmed that staff 1 had taken several children to public school in the center bus after this incident.
