Inspector Notes

An unannounced inspection was initiated on 04/07/2025 and completed on 04/23/2025 in response to a complaint received by the licensing office on 04/03/2025 relating to allegations of supervision and care & protection of children. The inspector reviewed two children's records on-site on 04/23/2025. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/02/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.
Staff 1 stated she failed to follow her usual practice of ensuring that child 1 was beside her during transition from outdoor play to inside of the facility. Child 1 has a diagnosed special need and , according to staff 1, is prone to wandering. This resulted in child 1 being left alone and unsupervised on the outdoor play area.
Plan of Correction: - Staff 1 received a retraining by management on LCG Face to Name Procedures immediately after the incident on 3/28/2025. Additionally, all staff will receive retraining on Face to Name Procedures and Transition Activities at our next staff meeting scheduled for 05/20/2025.
- A member of management will be present to observed every transition to and from the playground for the next 30 days.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.
Child 1, age 4 years, was left unattended on the outdoor play area for an undetermined amount of time which staff 1 estimated as a "couple of minutes" during the late afternoon of 03/27/2025.
Plan of Correction: - Staff 1 received a retraining by management on LCG Face to Name Procedures immediately after the incident on 3/28/25. Additionally, all staff will receive retraining on Face to Name Procedures and Transition Activities at our next staff meeting scheduled for 05/20/2025.

Standard 8VAC20-780-40-D
The license shall be posted in a place conspicuous to the public (? 22.1-289.011 of the Code of Virginia).
The license was not posted in the facility when the inspector arrived at 9:53 am on 04/07/2025.
Plan of Correction: - The license had fallen to the floor the morning of April 7th. Since the license could not be rehung immediately, the Center Director had taken the license into the office to reframe and rehang.
- The license was rehung in the foyer during the inspection on April 7th and remains there.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards.
Staff 1 stated she did not complete a name-to-face attendance count once children came inside from outdoor play on the afternoon of 03/27/2025 which resulted in a 4 year old children being left alone on the playground. Staff 1 stated that center supervision policy required an attendance count once children came inside from outdoor play.
Plan of Correction: - Staff 1 received a retraining by management on LCG Face to Name Procedures immediately after the incident on 3/28/25. Additionally, all staff will receive retraining on Face to Name Procedures and Transition Activities at our next staff meeting scheduled for 05/20/25.
