Inspector Notes

A monitoring inspection in response to a self-reported incident was initiated on November 15, 2022 and concluded on November 15, 2022. The report received by the department on November 3, 2022 included information of a serious injury sustained by a 14 month old child in care resulting in an avulsed tooth.

The Center's video recordings were observed, staff interviews were conducted, and the classroom was observed in response to the self reported incident.

Based on the preponderance of evidence, the investigation did not support the self report of non-compliance with regulations or law.
