8VAC20-780 Staffing and Supervision.

Inspector Notes

An unannounced monitoring inspection was initiated on 10/10/2024 and completed on 10/10/2024 in response to a self-report received by the licensing office on 10/09/2024 relating to Staffing and Supervision. The inspector reviewed 3 staff records on-site on 10/10/2024. This inspection included document review, interviews, tour of the facility, observations( to include video recordings) and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 10/08/2024 a fire drill was conducted at approximately 2:40pm. It was the end of nap time. Child #1 (2 year old) was not seen and left on their cot while the classroom was evacuated. There was confusion with the head count at the exit. As the Staff did their name to face check, approximately 20 feet into the breezeway from the corner of the classroom, the mistake was discovered. The child was immediately retrieved. The child was alone in the classroom for approximately 1 minute 18 seconds.
Plan of Correction: While teachers attempted to follow the trained procedure by counting at the door, miscommunication with one another resulted in a miscount. Fortunately, as trained, the teachers stopped to recount after a miscommunication, staying 20 feet away and within view of the room. However, teachers needed to take the time to discuss the mismatch in counts at the door prior to leaving the room, even in an emergency drill situation. We have conducted a full review of the incident, identified the importance of clear communication and instituted retraining of the team to improve our safety drill performance in the future. A faculty meeting discussion to address, clarify and review the supervision expectations will allow us to ensure the adherence to all parts of the procedure.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Plan of Correction: We very much regret this happened. We do practice regular fire drills to make sure all emergency procedures are followed. This incident showed us that some of our communication procedures needed to be reviewed and retrained. After a thorough investigation, it was noted that the teachers did perform a secondary headcount just outside the door of the classroom, within view of the room, to clarify the miscommunication in counting at the first headcount and demonstrated that this part of our procedures was an effective secondary protocol in noticing the mistake quickly. The teachers of the classroom have been retrained and appropriate disciplinary actions have been taken. A training for all the classrooms to reiterate supervision expectations during emergencies has also been held through a Faculty Meeting.
