A self-report was received by the Office of Child Care Health and Safety on 5/23/2023. An
unannounced inspection was conducted 5/30/2023. The inspector was on site from 4:10pm to 5:15pm.
The information gathered during the inspection did not support the self-report and therefore the self-report was determined to be invalid. Information gathered during the course of the investigation determined non-compliance with applicable standards or law. Violations unrelated to the complaint were documented on the violation notice issued to the provider.
