Inspector Notes

The Licensing Inspector initiated and completed an unannounced monitoring inspection on 6/5/2023. The inspector was on site from 10:20am until 1:50pm.

There were 135 children under the care of 23 staff. The ages of the children ranged from four months to five years old. The children were observed during indoor free play, outdoor play, lunch and naptime. Six children?s record and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on staff record review and interview, the center failed to obtain an Out of State child abuse and neglect search request for a new staff member.

Evidence:
Staff #4 began employment on 1/7/2023. Staff #4 has lived in another state within the past five years. The record for Staff #4 does not have a child abuse and neglect search request for the state of prior residence. Staff #7 stated it has been sent but not received back or checked on with the last 30 days.
Plan of Correction: HR will be send out an email to check on missing background check.
Standard 8VAC20-780-70
Based on staff record review, the center failed to ensure that each staff member had two or more references checked before employment or volunteering.

Evidence: Staff #1, who was hired on 08/06/2021 had reference checks done on 8/19/2021.

Staff #4, who was hired on 01/07/2023 had reference checks done on 1/09/2023.
Plan of Correction: All references will be checked prior to first day moving forward.
