During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/11/2025,as part of the licensure period. The on-site inspection began at 10:45am and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 107 children present and 22 staff. The inspector reviewed 10 children?s records and 7 staff records on-site.
This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
IF VIOLATIONS ARE CITED, AND A POC HAS NOT BEEN RECEIVED, ADD THIS PARAGRAPH: Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/21/2025. A POC submitted after this date will not appear on the public website.
Violations
3The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child 9, who has been enrolled over a year, had immunizations dated 3 weeks after enrollment.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was a physical examination in the record of child #7, It was dated almost two years after the 30 days requirement.
There was a physical examination in the record of child #9. It was dated 6 months after the 30 days requirement.
There was a physical examination in the record of child #5. It was dated 3 years after the 30 days requirement.
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.
The immunization record for child #7 was not signed or dated.
