Inspector Notes
An unannounced monitoring inspection was initiated on 3/28/2025 and completed on 5/5/2025 (with local agencies) in response to a self-report received by the licensing office on 3/26/2025 relating to staffing and supervision. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, and interviews.
After review with the Licensing Administrator, the inspection documents were updated on 5/12/2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administrations and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/20/2025. A POC submitted after this date will not appear on the public website.
Violations
4Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
The physical examination for Child #2 was obtained nine months after the first day of attendance.
The physical examination for Child #3 was obtained four months after the first day of attendance.
On 3/25/2025, Staff 1 sent Child 2 (age 4 years) to use a restroom that is located inside of the classroom and connects to the classroom next door. Child 1 (age 5 years) and Child 3 (age 3 years) separated from the group and entered the restroom, unnoticed by staff. During the approximately 5 minutes the three children were using the restroom not within sight supervision of staff, the children engaged in inappropriate activity (kissing). Intermittent sight supervision was not provided to assure the safety of the children.
The center?s policy is to ensure staff know where children are and what they are doing. Two children (age 3 years and 5 years) entered a restroom that was being used by another child (age 4 years). Staff were unaware the two children had separated from the group and were in the restroom without staff knowledge for approximately less than five minutes.
The record for Child 1, enrolled over a year, was missing the address for each emergency contact.
