Inspector Notes

An unannounced monitoring inspection was initiated on 8/18/2025 and completed on 9/24/2025 (with local agencies) in response to a self-report received by the licensing office on 8/12/2025 relating to forbidden Action. The inspector reviewed 1 children?s records and 1 staff records on-site on 8/18/25, additional record review was conducted on-site on 9/10/2025. This inspection included document review, tour of the facility, and interviews conducted on 8/18/2025, and 9/9/2025.

The inspection documents were reissued on 9/26/2025 due to a scrivener's error.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administrations and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/30/2025. A POC submitted after this date will not appear on the public website.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #3, who is working with children, has not yet completed this course which is overdue by 4 days.

Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center?s discipline policy for unacceptable discipline measures includes grabbing a child. On August 12, 2025, staff grabbed a four year old child on the arm leaving marks on the child's arm.
