Inspector Notes

An unannounced complaint inspection was initiated on 02/22/2022 and completed on 03/02/2022 with an in-person inspection.
Based on the information gathered which included staff interviews, a review of the physical plant & outdoor play areas a review of staff records and injury documentation this complaint is found to be not valid.

The inspector additionally reviewed records and interviewed staff for two serious injuries that occurred on 02/17/2022 and 02/28/2022. These injuries were self-reported by the facility as required.

There are no violations resulting from this complaint investigation.
