A monitoring inspection was conducted on 09/05/2023 to follow-up on violations cited during the 06/02/2023 inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all hazardous substances were shall be kept in a locked place using a safe locking method that prevents access by children.

(Repeat Violation) Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
1. The 2 composite play structures on the playground did not have the required resilient surfacing.
On the preschool playground there was a composite structure that had about 4 inches of mulch in the fall zones, were 6 inches is required. Grass and vegetation was observed growing in the fall zone areas of this piece of equipment.

Plan of Correction: The following corrective action was provided by the licensee:
Going forward the playground will be inspected by the safety captain and all deficiencies will be reported to management within 24 hours. Management will ensure all issues are handled and reported in a timely manner. Management will do periodic inspections to ensure compliance. Management will work with the DM to ensure the proper resilient surfacing is in place.
