Inspector Notes

An unannounced complaint inspection was initiated on 04/29/2025 and completed on 04/29/2025 in response to a complaint received by the licensing office on 04/26/2025 relating to allegations of non-compliance in the areas of physical plant, programs, special care provisions and emergencies and special services. The inspector reviewed 1 child's record and 3 staff records on-site on 04/29/2025. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegations of non-compliance in the area of physical plant and programs; therefore, these areas of the complaint are determined to be valid. Information gathered during the investigation determined non-compliance with applicable standards and the violations are documented on the violation notice issued to the program.
