Inspector Notes

An unannounced inspection was initiated on 08/25/2025 and completed on 09/12/2025 in response to a complaint received by the licensing office on 08/20/2025 relating to allegations of care and protection of children. This inspection included:

*Tour of the Infant classroom

*Observations

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards and the violation is documented on the violation notice issued to the facility.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/29/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.
1. Staff 1 fell asleep, twice, while holding an 8 month old infant in a rocking chair for approximately 6 minutes on 08/20/2025.
2. Staff 2 and staff 3 stated that staff 1 had told them she was tired and needed to go home. When they realized she had fallen asleep with an infant in her arms, they did not intervene immediately to ensure the safety of the infant, or notify management until staff 1 had fallen asleep a second time.
Plan of Correction: Staff 1 was immediately sent home once management was informed and was placed on administrative leave pending investigation. Staff 2 and staff 3 were coached on reporting any potential safety concerns immediately and ensuring children are safe at all times.
