Inspector Notes

An unannounced inspection was initiated on 01/12/2026 and completed on 01/30/2026 in response to complaints received on 01/06/2026 and 01/07/2026 relating to allegations in the areas of behavior guidance, care and protection of children,safety and diaper changing. This inspection included:

The preponderance of evidence gathered during the investigation does not support the allegation regarding inappropriate behavior guidance therefore, this area of the complaint is found to be not valid. The preponderance of evidence gathered supports the allegations in the areas of care and protection of children, safety and diaper changing therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/04/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.
The two-year class was combined with the three-year old class for three days the week of 01/05/2026. Staff report this disrupted the routine and overall safety of children in care as they could not implement planned activities due to the developmental difference between the age groups. Additionally, staff report the three-year old children were shoving and hitting the two-year old children and one child in the group has behavior challenges they did not feel sufficiently trained to handle with the addition of the younger children.
Plan of Correction: Management retrained staff on LCG's Behavioral Expectations Procedures including the importance of informing management of behavioral challenges and ensuring the care and protection of the children in their class. Moving forward, management will ensure that blended classes are able to implement planned activities according to their class schedule.
Standard 8VAC20-780-370-5
Infants shall be protected from older children.
Multiple staff report that infants have not been protected from toddlers when the groups are combined in the early morning. Multiple staff report incidents of toddlers sitting on, tripping over and hitting infants.
Plan of Correction: Management revised the schools schedule to minimize the need to combine toddlers with infants. Additionally, management completed a retraining on supervision to ensure infants are protected from older children. When the need to combine mixed age groups arises, management will consider the children's ages and developmental abilities when mixing ages.
Standard 8VAC20-780-500-B
For children younger than three years, diapers shall be changed on a changing table or countertop designed for changing.
Staff reported that two-year old children's diapers were changed, in the bathroom with the children standing up, for three days the week of 01/05/2026. The two-year old children were being cared for in the three-year old classroom which lacks a diaper changing table.
Plan of Correction: Moving forward all diapering children will be cared for in a room with diapering equipment and supplies.
