Inspector Notes

An unannounced inspection was initiated on 03/12/2026 and completed on 03/12/2026 in response to a complaint received by the licensing office on 03/10/2026 relating to allegations of inappropriate behavior guidance. This inspection included:

*Interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards, and the violation is documented on the violation notice issued to the facility.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/27/2026. A POC submitted after this date will not appear on the public website.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
When child 1, age 4 years, was acting out in a potentially dangerous manner, staff 1 used a phrase that was not constructure in nature in reacting to it, rather than using language that was more appropriate for redirecting the child.
Plan of Correction: The situation was thoroughly discussed with staff 1 to ensure that in the future, we focus our redirection on providing an appropriate behavior alternative for the child.
