Inspector Notes

An unannounced complaint inspection was initiated on 03/18/2026 and completed on 03/30/2026 in response to multiple complaints received by the licensing office on 03/06/2026, 03/11/2026, 03/12/2026 and 03/13/2026 relating to allegations in the areas of staff-to-child ratio, food service & nutrition, physical plant, supervision, and infant feeding. The inspector reviewed 1 staff record on-site on 03/30/2026. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegation regarding infant feeding therefore, this part of the complaint is determined to be valid. The preponderance of evidence gathered during the investigation does not support the allegations in the areas of food service & nutrition, physical plant, supervision and staff-to-child ratio therefore, these areas of the complaint are determined to be not valid.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/09/2026. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure there care and protection.
Staff 1 mistakenly fed 6 ounces of child 1?s bottle to child 2 on 3/6/2026 Both bottles were were labeled correctly with each child?s name.
Plan of Correction: Staff were retrained on the Infant Feeding Procedure. Management has also rolled out a new policy where staff must verbally read off the child?s name from the bottle out loud as they are preparing to feed the child.
