Inspector Notes
An unannounced complaint inspection was conducted on 3/22/23 to investigate an allegation regarding programs and special services. Based on evidence obtained through interviews with staff, the portion of the complaint related to programs was determined to be Valid. There was one violation issued as a result of this inspection. Based on multiple interviews, the facility failed to ensure that pillows shall not be used by children under two years of age. Evidence: interviews revealed that staff sometimes placed infant #1 (DOB: 10/24/2022) in the crib on a boppy pillow so that the infant would remain asleep.
Plan of Correction: All staff were retrained in safe sleep practices.
