Inspector Notes

An unannounced monitoring inspection was conducted from 1:30 pm - 3:45 pm with the center director. There were 79 children, ranging from seven months to 12 years of age, present with 15 staff supervising. Infant care was observed and older children were napping or playing quietly with age-appropriate toys. Five child records and five staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, required posted information and the fire and health inspections. It was reported that there was no medication on site, however staff were present with current certification in Medication Administration Training (MAT).

Based on a review of five child records, the facility did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #3 (date of first attendance: 3/1/2023) did not have documentation of a physical examination.

Standard 8VAC20-780-450-A
Based on observation, the facility failed to ensure that all cots used by children during the designated rest period have linens consisting of a top cover and a bottom cover. Evidence: approximately 9/16 children in the preK classroom were resting/napping on cots that did not have a bottom cover.
