An unannounced monitoring inspection was initiated and completed on 09/03/2025 with local agency DSS, in response to a self-report received by the licensing office on 08/25/2025 relating to behavioral guidance and forbidden actions. The inspector reviewed one children?s record and one staff record on-site.
