During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. AMENDED

An unannounced, on-site monitoring inspection was initiated on 2/19/2025 and completed on 2/19/2025. The on-site inspection began at 10:45 am and ended at 12:35 pm. The inspector reviewed compliance in the areas listed above. There were 62 children present, 7 staff, and 2 administrative staff. The inspector reviewed 5 children?s records, and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/27/2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3's updated TB screening but did not have verifying identification.

The staff-to-children ratios are required whenever children are in care.

During the inspection, In one of the 3-6 year old classrooms. There were 2 staff members and 23 children exceeding the required 1:10 ratio.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these
standards, and the terms of the current license issued by the department.

The center has a license that has an age range of 3 years to 12 years and 11 months. During the inspection, the center had 2 children present under the age of 3 years old. Both children have been enrolled in the center for over a month.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #4 has a documented food allergy noted on their physical examination form by a physician, however, there is no written care plan to include the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
