22.1 EARLY CHILDHOOD CARE AND EDUCATION
Inspector Notes
A monitoring inspection was conducted on June 17, 2022 and concluded on June 17, 2022. On June 17, 2022, the licensing inspector was onsite from approximately 10:30am until 1:30pm. The director assisted with the inspection. There were 58 children present, ranging in ages from five months to seven years, with ten staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and eight staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence:
1. The record of staff #1, hired 04/25/2022, indicated staff #1 had resided in another state in the past five year. The record did not contain documentation of a abuse and neglect check request made within 30 days of employment.
2. The director stated an out of state abuse and neglect check had not been requested for staff #1.
Evidence:
1. The record of staff #1, hired 04/25/2022, had documentation of a negative tuberculosis screening dated 04/29/2022, exceeding the date of employment.
2. The record of staff #6, hired 04/15/2022, had documentation of a negative tuberculosis screening dated 06/12/2022, exceeding the date of employment.
3. The record of staff #7, hired 06/03/2022, had documentation of a negative tuberculosis screening dated 02/11/2022, exceeding 30 days prior to employment.
4. The record of staff #8, hired 06/17/2022, did not have documentation of a negative tuberculosis screening at the time of employment or prior to contact with children. Staff #8 was observed working in the three year old classroom.
Evidence:
1. The record of staff #3, hired 03/07/2022, did not contain documentation of VA preservice training exceeding 90 calendar days from employment.
2. The record of staff #5, hired 01/24/2022, did not contain documentation of VA preservice training exceeding 90 calendar days from employment.
3. The director stated the two staff had not completed the required training.
Evidence:
The prescription medication for child #2 was not in its original box nor did it have the pharmacy label with the dosage, time or times to be given.
Evidence:
1. The record of staff #7, the only staff assigned and present in the school age classroom, did not have documentation of current CPR and first aid certification.
2. The director stated staff #9, the only staff assigned and present in the toddler classroom, did not have current CPR and first aid certification.
3. The director stated staff #10, the only staff assigned and present in the pre-k classroom, did not have current CPR and first aid certification.
