Inspector Notes
A self-reported incident regarding forbidden actions was received by the Office of Child Care Health and Safety on June 20, 2023. The inspector initiated the investigation with a phone interview on 06/20/2023. An unannounced monitoring inspection was conducted on July 10, 2023. The inspector was on site from 11:00 am until 12:15 pm. Interviews with staff were conducted, observations were made at the center and documentation was reviewed including one staff record and one child?s record. An additional interview was conducted off site on July 25, 2023, with other local agencies. The preponderance of the evidence gathered during the investigation does not supports areas of noncompliance with the licensing standards related to the forbidden action, therefore the allegation was determined to be not valid.
