Inspector Notes
An unannounced complaint inspection was conducted in response to allegations received by the department on March 4, 2024, regarding programing. The inspector was on site from approximately 1:30 pm until 5:20 pm. Interviews with staff were conducted and observations were made. Daily infant sheets were reviewed and child?s record. The preponderance of evidence gathered during the investigation supported a portion of the allegation of non-compliance with standards or law and therefore was valid.
This inspection report was amended on May 7, 2024.

Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

2Evidence:
1. The daily record for child #1, 3 months in age and unable to turn over himself, did not contain documentation of time spent on his stomach for four days during the period from February 20, 2024, through February 29, 2024.
2. On February 21, February 23rd, February 28th, and February 29, 2024, there was no documentation of child #1 having time on his stomach.
Evidence:
The daily sheet for child #1 (3 months in age), dated 02/21/2024 through 02/29/2024, did not contain the amount of time child #1 spent on his stomach each day.
