Inspector Notes

An unannounced complaint inspection was conducted in response to allegations received by the department on March 4, 2024, regarding programing. The inspector was on site from approximately 1:30 pm until 5:20 pm. Interviews with staff were conducted and observations were made. Daily infant sheets were reviewed and child?s record. The preponderance of evidence gathered during the investigation supported a portion of the allegation of non-compliance with standards or law and therefore was valid.

This inspection report was amended on May 7, 2024.

Based on review of documentation, the center did not ensure infants, who cannot turn themselves over and are awake, were placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns.

Evidence:

1. The daily record for child #1, 3 months in age and unable to turn over himself, did not contain documentation of time spent on his stomach for four days during the period from February 20, 2024, through February 29, 2024.
2. On February 21, February 23rd, February 28th, and February 29, 2024, there was no documentation of child #1 having time on his stomach.
Plan of Correction: Per the director: Center will provide tummy time as appropriate to each individual child's age and developmental ability.

Based on review of documentation, the center did not ensure the daily record for each infant, who is awake and cannot turn over by themselves, contained the amount of time spent on their stomachs.

Evidence:

The daily sheet for child #1 (3 months in age), dated 02/21/2024 through 02/29/2024, did not contain the amount of time child #1 spent on his stomach each day.
Plan of Correction: Per the director: All tummy time will be documented via parent communication app each time it is offered.
