Inspector Notes

A renewal inspection was conducted on 9/8/2022. There were a total of 35 children with 6 staff members present. Children were observed during nap time and bottle feeding.. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-4
Based on a review of staff records, the center did not obtain central registry checks for any state in which the individual has resided in, in the preceding five years.
Evidence:
Staff A (date of hire 8/5/2021) did not have an out of state central registry check on file.
Plan of Correction: We will request the out of state background check.
Standard 8VAC20-780-160-A
Based on a review of staff files, each staff member did not submit documentation of a negative tuberculosis screening within 30 days of employment.
Evidence:
Staff C (date of hire 5/23/2022) had TB results dated 03/16/2022.
Plan of Correction: We will make sure everyone has it within 30 days or employment.
Standard 8VAC20-780-240-A
Based on a review of staff records, the VA department of education- sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
Staff C (date of hire 5/23/2022) had the orientation course dated 9/5/2022.
Plan of Correction: We will make sure every employee has it within 90 days of employment.
Standard 8VAC20-780-440-E
Based on observation, there were not at least 12 inches of space between occupied cots.
Evidence:
In the toddler room, two of the cots near the windows were touching at the top. The children were head to head. In the Prekindergarten room 4 of the cots along the wall leading outside were less than 12 inches.
Plan of Correction: We will move cots further apart. At least 12 inches.
Standard 8VAC20-780-450-A
Based on observation, cots used by children, did not have linens consisting of a bottom cover.
Evidence:
Two of the cots occupied by children in the Prekindergarten classroom did not have bottom sheets.
Plan of Correction: We will make sure children have a bottom sheet.
