Inspector Notes

An investigation was conducted today in response to a self-reported incident received by our office regarding the care and protection and medications for a two-year-old, and supervision for two children on the playground in the Pre-K classroom.

Interviews were conducted and documentation were reviewed. There was not a preponderance of evidence to support the allegation regarding supervision; however, the complaint was deemed valid with regard to medications for a two-year-old child. The complaint related violations and
one non-related violation are identified in the Violation Notice.

Based on interviews and staff records, records and reports on children and staff required by
this chapter were not maintained and made accessible for two years after termination of
services or separation from employment unless specified otherwise.
Evidence:
1. Child #1 did not have medication record for review during the inspection.
2. During the child's enrollment, Staff #1 stated child was prescribed Benadryl and Epi-pen.
3. Staff #1 stated child did not have Benadryl and Epi-pen onsite to take in care of
emergency.
Plan of Correction: Copies of medication paperwork made and placed into the files. List of all medication with expiring date added to medication policy.
