During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 06/13/2025. The on-site inspection began at 9:30 AM and ended at 11:20 AM. The inspector reviewed compliance in the areas listed above. There were 114 children present with 24 staff. The inspector reviewed 8 children?s records and 8 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.031-A
A child day center operated or conducted under the auspices of a religious institution, that has chosen to be exempt from the licensure requirements of this chapter, shall post the fact that it is exempt from licensure in a visible location on the premises.
The facility did not have posted that the facility is exempt from licensure.
Plan of Correction: Facility response:
The notice that the facility is exempted from licensure will be immediately posted.
Standard 22.1-289.035-A
Background checks to include results of a central registry search shall be conducted within the past five years. The most recent central registry search results obtained for staff 1 and staff 7 were obtained over six years ago.
Plan of Correction: Facility response:
repeat searches of the central registry will be obtained for staff 1 and 7 immediately. Background checks for all staff will be tracked to ensure that repeat checks are obtained within every five years.
Standard 22.1-289.035-B-2
Results of a fingerprint-based criminal history determination shall be obtained prior to the first day of staff employment. Staff 4 was employed two days prior to receipt of a fingerprint-based criminal history determination was received.
Plan of Correction: Facility response:
In the future, results of a fingerprint-based criminal history determination will be obtained prior to the hire of new staff.
Standard 22.1-289.035-B-4
The child day center shall obtain a copy of the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Results of a sex offender registry search and a search of the child abuse and neglect registry were not obtained from a state in which staff 5 lived within the past five years.
Plan of Correction: Facility response:
A central registry search will be obtained from the state in which staff 5 has lived. Background checks will be obtained from any state in which an applicant has lived prior to the employ of staff.
