Inspector Notes
A mandated monitoring inspection was conducted on April 13, 2023 from 10:00 A.M.-1:00 P.M. There were 50 children present, ranging in ages from 1 year 8 months to five years of age, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and eight staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
1Evidence:
1. Child #1 had an emergency medication, Albuterol, on site at the center. The medical authorization form from both the parent and physician expired on 04/11/2023. The medication had not been administered.
2. Staff #1 verified the dates on the authorization form.
3. Staff #2 verified that they had not obtained updated authorization forms.
